Introducing the electronic Summary Care Record

“In March this year NHS Connecting for Health launched a public information programme in the first of a series of ‘early adopter’ Primary Care Trusts (PCTs). Patients in Bolton became the first in England to receive information telling them about the introduction of the new electronic Summary Care Record. The Summary Care Record forms the national element of the NHS Care Records Service which will see an electronic record being created for every person in England who does not opt-out by 2010.”

Rise of the PHR

“It isn’t nearly as comprehensive and trustworthy as the ideal electronic health record, but what’s called the personal health record has its place.”

Personal Health Records: Definitions, Benefits, and Strategies for Overcoming Barriers to Adoption

“Recently there has been a remarkable upsurge in activity surrounding the adoption of personal health record (PHR) systems for patients and consumers. The biomedical literature does not yet adequately describe the potential capabilities and utility of PHR systems. In addition, the lack of a proven business case for widespread deployment hinders PHR adoption. In a 2005 working symposium, the American Medical Informatics Association’s College of Medical Informatics discussed the issues surrounding personal health record systems and developed recommendations for PHR-promoting activities. Personal health record systems are more than just static repositories for patient data; they combine data, knowledge, and software tools, which help patients to become active participants in their own care. When PHRs are integrated with electronic health record systems, they provide greater benefits than would stand-alone systems for consumers. This paper summarizes the College Symposium discussions on PHR systems and provides definitions, system characteristics, technical architectures, benefits, barriers o adoption, and strategies for increasing adoption.”

Implementing EHRs Requires a Shift in Thinking

“PHRs—the building blocks of EHRs—may be the quickest path to the fulfillment of disease management.
During the past 20 years, physicians have seen amazing advancements in technology and have witnessed their beneficial impact on the health and the lives of hundreds of patients. They have also seen the amazing impact that sound information can have on a patient’s health and on the decisions made regarding that patient’s care. While society as a whole has never been more connected, more integrated, more technologically advanced or more global, healthcare still has some fairly old-fashioned mores that must be challenged and reshaped to truly advance electronic health records (EHRs) on a large and sustainable scale.”

EMR or EHR: Is There Really a Difference?

“starting a conversation around it to raise awareness that the EHR is broader than the EMR and that the government’s vision is for every American to have their own electronic record. It’s helpful to think of the EHR in these broader terms. It’s patient centered and that needs to be our ultimate destination.”

Electronic Medical Records vs. Electronic Health Records: Yes, There Is a Difference

“Many people in the US healthcare industry, our government, and the press use the terms electronic medical record (EMR) and electronic health record (EHR) interchangeably. However, these terms describe completely different concepts, both of which are crucial to the success of local, regional, and national goals to improve patient safety, improve the quality and efficiency of patient care, and reduce healthcare delivery costs. EHRs are reliant on EMRs being in place, and EMRs will never reach their full potential without interoperable EHRs in place. It s important to understand the differences, and to reduce confusion in the market.”

The Role of the Personal Health Record in the EHR

“Health information technology will be integral to the US transformation to a safer, more efficient, consumer-driven healthcare system, and the personal health record (PHR) will be a valuable asset to individuals and families, enabling them to integrate and manage their healthcare information through secure, standardized tools. It is imperative that patients, healthcare providers, and payers work together to develop a PHR model. Establishing a common data set is a vital starting point.”

Evolving Conceptions of Personal Health Records

“This is a time of lively experimentation in which there are many models of personal health records. Two broad types of PHRs are evolving in the private and public sectors, distinguished primarily by their relationship to EHRs. One is a patient-facing extension of clinician-controlled EHRs; the other is not routinely linked to the patient’s EHRs.”
